Athletic Enrollment APPRYSE

Appryse is a web-based replacement to the paper-based system previously used to track medical history, contact information, release forms and doctor’s physical forms. Parents and guardians are able to enter all information online and upload or fax doctor physical forms to Appryse.

You will no longer need to bring or send school physical forms or blue forms to the nurse.

In the event of a medical emergency, trainers and coaches will use the system to automatically notify you of an emergency, symptoms, and/or care, etc. via phone, email, and text message.

We feel it will make this year much easier for parents and guardians, coaches, and most importantly, increase the safety of our students.
